Mee Derby can help you in your job search, we will ask you to complete
our Pre-Search Assessment, shown below. The Assessment asks seven
sets of questions directly related to your job search. Your answers
will guide Mee Derby to find the best possible positions for your
consideration. Included in the Assessment is a Release Form for
checking references.

Thank you for contacting Mee Derby & Company during your current
job search. Mee Derby specializes in placing Executives, Managers,
Salespeople, and Recruiters in Staffing Services, Professional Services,
and Human Capital Management.
After conducting thousands of searches for hundreds
of national, regional and local companies since 1988, our clients
have come to rely on Mee Derby to provide them with well-qualified,
carefully screened candidates. We know from experience that this
Pre-search Assessment will help us find the best candidates for
the positions our clients are trying to fill.
To expedite our interview process, we would like
you to complete the information below. Be assured that we maintain
the strictest privacy and confidentiality in our search work and
that all material and information provided by you will be secure.
Please return your answers to the following questions,
and the completed Reference Release Form, to Eliza Deang via email
or fax (301-320-7786) as soon as possible. We look forward to working

- What is motivating your decision to make a
job change?
- Describe your recent search activities. In
order for us not to duplicate your on-going efforts, please include
any information about interviews and/or offers you have declined
or have pending, as well as the types of positions you are considering.
- Describe in detail your ideal next position.
What activities would you be responsible for, whom would you report
to, what types of projects would you handle? The more detail you
provide the more help Mee Derby can be to you in finding the type
of position you are seeking.
- Where are you willing to work? How far are
you willing to travel to and from work? Are you willing to relocate?
If yes, to what area?
- What is your total compensation
package? Please include base, bonus, incentives, stock, etc, and
list each separately. List W-2’s for last 3 years with base
salaries for each. What type of package do you expect in your
next move? Please make sure that you can substantiate the information
provided regarding earnings that you will indicate here. Many
of our clients will request a hard copy of the W-2’s to
confirm the salary information.
- When you notify your present employer that
you are leaving, would you expect a counter-offer? Have you ever
been given a counter offer? Did you accept it or turn it down?

- Do you have a non-compete
and/or non-solicitation contract/agreement with previous employment?
If yes, will you be able to provide us a copy if requested by
the client? What are the limitations and terms of that non-compete?

REFERENCES RELEASE FORM
All Mee Derby clients require business, peer,
and client references from prospective candidates. Signing the disclosure
statement below allows us to check your references, which will be
done only when we deem it necessary to further the placement process
or to verify information you have given us. Reference information
is always kept in the strictest professional confidence. Any references
that you fear might jeopardize your current employment will not
be conducted unless and until we have your approval.
Please provide us with the names, telephone numbers,
and titles of at least three references - ideally your current and
former Managers, and one client. Briefly describe your relationship
to these references. If you are working, we realize that in most
cases you cannot use your current supervisor as a reference. Supervisors
from a previous employer are acceptable, as are trusted peers in
your current company. You will be notified before any references
are contacted, or your resume is presented to any of our clients.
